Blue Flower Health provides services performed by a licensed Registered Nurse (RN) in the State of Missouri.

Services may include:

• Educational wellness guidance
• General explanation of laboratory results
• Assistance coordinating laboratory testing through third-party providers
• General health education and support

All services are provided within the legal scope of practice for a Missouri Registered Nurse.

2. RN Scope of Practice Disclosure

You acknowledge and understand that:

• Services are provided by a Registered Nurse, not a physician or advanced practice provider
• Under Missouri law, an RN does not diagnose medical conditions
• An RN does not prescribe medications or treatment plans
• Services are limited to education, support, and general wellness information

3. No Provider-Patient Relationship

Use of this service does not establish a physician-patient relationship.

Use of this service establishes only a limited educational nurse-client relationship that:

• Does not include medical diagnosis or treatment
• Does not replace care from a licensed healthcare provider

4. Laboratory Testing Disclosure

If laboratory testing is purchased:

• Testing may be coordinated through independent third-party laboratories and authorized providers
• Blue Flower Health does not act as the diagnosing or treating provider
